How to send a referral
Email or call us with your referral. Fax number coming soon — if you’d prefer to fax, reach out and we’ll arrange it. A written prescription or referral for occupational therapy is required and should include:
- Patient name, date of birth, and contact information
- Diagnosis or reason for referral (e.g., “OT evaluate and treat — post-concussion syndrome”)
- Relevant reports, imaging summaries, or vision exam findings
- Any precautions or activity restrictions
We’ll contact the patient or family directly to schedule and handle benefits verification.
What comes back to you
- Initial evaluation report with findings, goals, and plan of care
- Progress updates at re-evaluation intervals and on request
- Discharge summary with outcomes and recommendations
- Prompt communication about any concerns or changes in status
For vision referrals, we document functional progress so you can correlate it with your clinical findings.
